Hospital Discharge and Support

It may be that there has been an accident or an unexpected discharge from hospital. If this happens, please contact Acquire Care and see how we can help you.

If you or a loved one has been discharged from hospital but feel that help is still needed, then one of our friendly, helpful carers can make everything seem easier to cope with. Perhaps, the hospital will not allow you home unless there is help at home. If this is the case, then please give us a call. A hospital stay can be a difficult time for both you and those close to you. No one likes to stay in hospital and always hopes the visit will be as short as possible. Acquire Care can help by being at home when you are discharged.

The type of help needed when you are discharged will depend on why you were in hospital. Acquire Care can help with the following but if you have other needs please ask:

  • Bathing
  • Dressing
  • Eating (are there diet restrictions, e.g., soft foods only? Certain foods not allowed?)
  • Personal hygiene
  • Grooming
  • Toileting
  • Transfer (moving from bed to chair)
  • Mobility (includes walking)
  • Using certain special equipment
  • Co-ordinating and overseeing medication
  • Household chores

As with all our Clients, a specialised care plan will be arranged and implemented so that everyone is well organised and knowledgeable. This care plan will be regularly checked and adapted to your health progress at home.

Services

There are a range of effective, helpful and friendly care services that can be provided. These are all tailored to the Client’s needs. The services include personal care and support through to shopping, doing laundry, collecting prescriptions and even walking the dog.

Costs and Funding

The cost of any long term care, whether Hourly Care or Live-in Care, can be a concern. The cost varies according to the amount of care and support that is supplied. There is a variety of funding which may be accessible to you to help with the costs.

Training

Training our carers to the highest standard possible is an important part of Acquire Care’s ethos. By having a specialist trainer with in-house training facilities, we ensure that all our carers are fully trained and vetted as required by the Care Quality Commission.
